2. Student of BS programs of University of Haripur.

Requirements/Documentation:
Students need to attach the following attested photocopies along with the application form.
1. Detail Marks Certificate (DMC)
2. Domicile Certificate/Form-B of the student
3. CNIC of Education Employee
4. Recent pay slip of Education Employee
5. Slip of EEF Registration Number (procedure for registration can also be seen on the above link at Sr. No. terms and condition 3)
Before Submission:
After filling and completing application form,
1. Student may sign himself / herself on the application form.
2. Sign and stamp from relevant HOD below Serial number 13.
3. Parent signature is also mandatory below serial number 26.
4. Signature of relevant District Education officer below serial number 26.
5. Documents will be sent by the student himself / herself via TCS / LCS / Pakistan post on the address given below:
